Expression of Wish form
The Expression of Wish form enables you to indicate to the Trustee how you wish the benefit to be paid on your death.

Change of bank details
If you are receiving a pension from the scheme, you should inform Barnett Waddingham if you wish to change the bank account your pension is paid into via the Clarity self-service site or this form.
Authorisation form
If you’d like Barnett Waddingham to deal directly with either your financial adviser or a potential receiving arrangement, you should inform them by using this form.
Internal dispute resolution procedure
Most disputes are settled quickly and informally after consultation with the Trustee. However, the scheme has a formal internal dispute resolution procedure (IDR procedure) for any unresolved disputes. Further details on the IDR procedure can be found in this document.

- the investment options in which DC members invest
- the requirements for processing DC financial transactions
